Welcome to the Clay Church Resource request page.  If you would like to request the services of the resource team members or the use of Clay Church property please complete the appropriate form(s). You will be contacted by a member of our Resource Team to verify your request and gather any additional information that is required before approving your request.  Thank you for your assistance & patience during this process.

Facilities Request Form

This form is used to request the use of any space within a Clay Church facility.  If you need Clay Church to communicate this event as well then the Clay Church Communication Request will also need to be complete.  NOTE: If you are requesting an event time that is less then 48 hours from the request time please contact Tom Kauble ASAP at 574-261-9398!

Communication Request Form

This form is used to request Clay Church to advertise or promote your event. This form is also to be used if you need Graphic Design work or hard copy print work completed.  Please ensure contact information is correct as this person will be responsible for approving all communication materials before publishing.  Click Here for the Clay Church Communication Guidelines.

Audio/Visual Support Request Form

This form is used to request the use of any Clay Church Audio/Visual equipment or to request the use of an A/V Team Member to develop a picture, video or audio media.

 
 

Maintenance Work Request Form

This form is used to request any Maintenance work on Clay Church property. If this is an emergency request and needs attention within the next 24 hours please contact Jean Womer @ 574-286-8367 or Tom Kauble @ 574-261-9398 as soon as you have completed the online request.

Vehicle Request Form

This form is used to request any Clay Church Vehicle. Valid drivers lisence required.

Child Care Request Form

This form is used to request Child Care services at a Clay Church facility. A separate Clay Church Facilities Request Form is required to reserve space for your event being held at a Clay Church Facility.

Wedding Request Form

This form is used to request the use of any Clay Church Facility for a wedding.  Weddings can be scheduled 15 months in advance of the wedding date.  $100 reservation fee required to schedule all weddings.
